In 1990, Community Health Centers, Inc. (CHC) received Federal funding to provide health services to Utah's migrant and seasonal farmworker population. With this funding, CHC provides medical, dental, health education, and outreach services through Utah Farm Worker Program (UFWH). UFWH also refers patients to other existing health care providers and resources.
UFWH is based on the primary care model. The program maintains a year-round clinic at the Brigham City site in northern Utah's Box Elder County. UFWH also provides regular dental services two days a week and one evening a month at the Brigham City site. Additionally, UFWH supports year-round primary care of farmworkers through contracts with Midtown Community Health Center in Ogden and Mountainlands Community Health Center located in Provo. Emergency dental and other specialty care is provided through contract with community providers.

CHC accepts most private insurance policies, Medicaid, Medicare and Utah CHIP.  Patients without insurance may qualify for a "sliding fee" discount based on income, family size and an ability to pay.  Co-payments are due the day of service unless prior arrangements have been made.  For more information concerning payment options click here
