PUBLIC NOTICE TO ARCHITECT FIRMS
REQUEST FOR PROPOSALS
Community Health Centers, Inc., is requesting proposals from architect firms to provide professional services
for a medical clinic design, schematic and construction drawings/specifications, with potential for project
management including bid management and construction oversight the following project:
MIDVALE/FT. UNION COMMUNITY HEALTH CENTER

Your proposal must be received no later than 5 p.m., MST, Friday September 10, 2010.
Minority and women owned firms are encouraged to submit a RFP. No person shall be

funds through HRSA. Attention is particularly called to the requirements as to conditions of employment to be

as amended and Contract Work Hours Act, Clean Air Act, Byrd Anti-Lobbying Amendment, Debarment and Suspension
(E.O’.s 12549 and 12689) and Safety Standards Act requirements.

Proposals will be evaluated on the basis of written materials. CHC will contact the highest ranking firm and
enter negotiations. If CHC is unable to negotiate a satisfactory agreement, the second ranked firm will be
contacted. CHC reserves the right to accept no bids. The costs of your bid preparation are not reimbursable.
