We welcome everyone to become part of our family at the Community Health
Centers, Inc. We are committed to providing affordable, quality health
care to people throughout Salt Lake County and surrounding communities.
At CHC, we make every effort to provide care for as many people as
possible in the communities we serve; however, the need often exceeds
available funding.
Payment Methods
Payment of co-pays and certain other fees are expected at time of
service. Uninsured patients are also expected to pay appropriate
co-pays at time of service. CHC accepts cash, Visa/Master Card, your
personal check from a local bank, and various forms of insurance. Below
is a list of insurance policies which CHC accepts. Most private
insurance policies are accepted at CHC. If your private insurance is
not listed below, please ask if we accept your insurance at the time you
make an appointment.

On-line Payments
Patients can make secure on-line payments using a checking account,
savings account or a major credit card. Please have your CHC account
number and checking/savings account numbers or credit card information
before you visit the secure on-line payment page.
Discounted Fee Program
Community Health Centers, Inc has a sliding fee schedule to discount the
cost of medical or dental care for those patients with incomes below 200
percent of the federal poverty line. All eligible patients who apply
for the sliding fee discount program will need to provide proof of
income. All eligible patients participating in the sliding fee discount
program are still expected to pay appropriate co-pays at time of

Frequently Asked Questions About Our Discounted Fee Schedule
How do I apply?
Applications for the Discount Program are available at each health
center. You may complete an application at any time; however, your
application must be approved prior to your appointment in order to
receive the discount. You must provide proof of income for CHC’s
sliding fee discount program. Only one application is needed for your
entire household - make sure each person that you want included in the
sliding fee discount program is included on that form.
What information do I need to complete the application?
Patients must bring proof of income for EACH member of the household
that is included on the application. You will not be approved for this
discount without proof of income. You will be responsible for the full
fees associated with your visit prior to approval.
How often do I apply?
You must apply once a year or when your household income or family size
changes. Once your application expires you must reapply. You will be
responsible for the full fees associated with your visit prior to
re-approval.

Any personal information and/or health related information is protected
