of service and support while securing patient information from any type

Our Commitment to Integrity
It is the policy of Community Health Centers, Inc. to conduct
business according to the highest ethical standards to maintain the complete confidence
of our volunteers, employees, and donors. Volunteers and employees must conduct their personal
affairs and manage their business transactions in a manner that does not result in adverse
criticism from the public or in any way damage CHC’s reputation as a responsible non-profit
agency.
CHC has selected EthicsPoint to provide you with a compliance hotline
as a way to report concerns about practices that may jeopardize the integrity of our
organization and people. You have the ability to submit a report to EthicsPoint
independently when and where you choose. This hotline service is reserved for reporting
the following issue types addressed in CHC’s Standards of Conduct: Conflicts of Interest,
Improper Lobbying or Political Contribution, Breach of Confidentiality, Improper
Gifts/Gratuities, and Violation of Procurement Standards. All other concerns must be
addressed directly with your supervisor.
All submitted reports will be treated as confidential. The report may
be submitted anonymously if you choose to do so; however, CHC cannot guarantee complete
anonymity in all cases. Upon completing the report you will be give a report key and
password that will allow you to check the status of your report within 5-6 days.

spirit of all applicable laws and regulations. CHC expects its volunteers and employees
to use good judgment, to apply the highest ethical standards and to refrain from any
form of illegal, dishonest, or unethical behavior. It is your responsibility to ensure
