Introduction
Community Health Centers is one of the leading not-for-profit healthcare providers in the state of Utah.  As a leader in our community, we strive to ensure that all facets of our organization contain the highest level of service and support while securing patient information from any type of unlawful disclosure.  As a result, below is an explanation of what our policy is regarding any type of information gathered from our online presence.

Website Tracking
All data collected by the sever run by the company hosting our website is used for their billing and authoring purposes.  Any data collected on the behalf of Community Health Centers, Inc. is for determining daily, monthly and yearly traffic to and from www.chc-ut.org.  Information collected on the website will not be disclosed to third parties under any circumstances other than to aid in the investigation of criminal activity.
How We Use Cookies

Our Commitment to Integrity
It is the policy of Community Health Centers, Inc. to conduct business according to the highest ethical standards to maintain the complete confidence of our volunteers, employees, and donors. Volunteers and employees must conduct their personal affairs and manage their business transactions in a manner that does not result in adverse criticism from the public or in any way damage CHC’s reputation as a responsible non-profit agency.
CHC has selected EthicsPoint to provide you with a compliance hotline as a way to report concerns about practices that may jeopardize the integrity of our organization and people. You have the ability to submit a report to EthicsPoint independently when and where you choose. This hotline service is reserved for reporting the following issue types addressed in CHC’s Standards of Conduct: Conflicts of Interest, Improper Lobbying or Political Contribution, Breach of Confidentiality, Improper Gifts/Gratuities, and Violation of Procurement Standards. All other concerns must be addressed directly with your supervisor.
All submitted reports will be treated as confidential. The report may be submitted anonymously if you choose to do so; however, CHC cannot guarantee complete anonymity in all cases. Upon completing the report you will be give a report key and password that will allow you to check the status of your report within 5-6 days.
