Can I see a provider today? Top
For acute care (sickness, minor injury), yes. For well checks, immunizations, and chronic health conditions, appointments are preferred.

What if I have no insurance? Top
You have two choices: 1) You might qualify for a discounted sliding fee program based on your income, or 2) You can pay the full cost of your visit.

What if I have no insurance? Top
If you do not need to see a provider today, we will schedule an appointment in the near future to determine your eligibility for various discount programs. If you need an appointment today, we will have you come in early to meet with our intake professionals and try and get you qualified today.

In order to qualify for a sliding fee program, you will need to bring: A pay stub showing the previous month's income, a photo ID, Social Security card, birth certificates for children, car registration and a bill with your name on it. Everyone may not need all of this and the list is tailored to meet the individual.

What if I am unable to pay today? Top
We ask that patients pay their co-pay (for insured or on sliding fee) or a the full fee (for self-pays) at the time of service. If you are unable to pay in full or make a partial payment, you can still be seen for acute care.

Do you take credit cards? Top
We accept Visa, Master Card, American Express and debit cards.

What insurance do you accept? Top
Altius, CHIP, IHC Health Select, Medicaid APM, PCN, Baby Your Baby, CHIP AFC Molina, CHIP PEHP, EMED, Health U - Medicaid and the Healthy U Flex Care Plan.  Contact our billing department for other insurance coverage.

Do you accept Medicaid and Medicare? Top
Yes, these are welcomed at Community Health Centers, Inc.

Can I get my prescription refilled today? Top
There is a 24 to 48 hour refill policy. Our policy is to have the pharmacy call us for the refill.

Can I speak to a provider directly? Top
Our protocol is to give your message to the provider's medical assistant (MA), who then passes the message on to the provider. Due to busy schedules, it may take time for you to receive a call back. Call backs are made between patient visits, at lunch, or by the end of the day.

Can I speak to someone about my bill? Top
Yes, we have both English and Spanish speakers who can help you with billing questions. Do not hesitate to call (801) 412-6939 if you have a question.

Do you do x-rays? Top
No.  We have arrangements with local x-ray providers.  A sliding fee schedule and a voucher program has been established based on income and family size.
