We are committed to providing affordable, quality health care to people throughout Salt Lake County and surrounding communities. At CHC, we make every effort to provide care for as many people as possible in the communities we serve. We utilize every funding stream available to provide services to as many patients as possible and are constantly looking for new opportunities to increase available services in our neighborhoods. We work with patients to find a plan that fits their budget.

 

Payment Methods 
Payment of co-pays and certain other fees are expected at time of service. Uninsured patients are also expected to pay appropriate co-pays at time of service. CHC accepts cash, Visa/Master Card, your personal check from a local bank, and various forms of insurance.

 

Most private insurance policies are accepted at CHC. If your private insurance is not listed on the below, please ask if we accept your insurance at the time you make an appointment.

 

  • Utah Medicaid (including Emergency Medicaid)

  • Healthy U Plans

  • Select Community Medicaid (previously Select Access)

  • Molina Medicaid

  • Health Choice Medicaid

  • Utah CHIP

  • Medicare

  • PCN

  • PEHP Plans

  • Select Health Plans (IHC)

  • Most private insurance plans

Discounted Fee Program 
CHC ensures that no one will be denied access to health services due to their inability to pay. CHC offers a schedule of sliding fees to discount the cost of medical or dental care for those patients with incomes below 200 percent of the federal poverty line. All eligible patients who apply for this discount program must provide proof of income. Patients are expected to pay appropriate co-pays at time of service.  Your bill will always be at least $35 for a medical visit and $55 for a dental visit.

Acceptable Forms of Income Verification

  • IRS Form W-2 or 1099

  • Most recent tax return

  • 2 months of pay stubs for you & your spouse

  • Certified letter of income from your employer(s)

  • Unemployment benefits statement

  • Social Security, disability or pension benefit statements

  • Documentation of other government assistance

Applications for the Discount Program are available at each health center. You may complete an application at any time; however, your application must be approved prior to your appointment in order to receive the discount. You will be responsible for the full fees associated with your visit prior to approval.

Patients must bring proof of income for EACH member of the household who is included on the application. You will not be approved for this discount without proof of income. You must re-apply for the discount program each year or sooner if your household income or family size changes.
